The Hospice philosophy
centers on the belief that quality of life is as important as
quantity of life. Each person has unique needs and strengths,
our goal is to do whatever it take to make them feel confident and
comfortable enough to live in their home, whether it's a private
residence, an AFC Home or a nursing home.
Hospice treats the person, n ot
just physically, but also emotionally, psychologically and
spiritually, recognizing that a person is much more than just a
physical body.
Caring for a terminally ill loved one
can have a tremendous physical and emotional impact on a
family. Hospice can provide the assistance a family needs to
care for their loved one. |

Hospice provides an
interdisciplinary team approach to care including: |
Nurses
who implement the plan of care, access the patient's needs and
keep the physician informed of the patient's condition.
Home Health Aids who assist with personal care
and activities of daily living.
Social Workers who assure that the patient and
family have everything they need to help them cope with
terminal illness
Therapists who provide nutritional, physical,
occupational or speech therapy if ordered by a patient's
physician.

Medical Director who works with the patient's own
physician in developing and guiding the plan of care.
Clergy who offer spiritual support and guidance,
personally or in conjunction with a family's minister of
choice.
A Bereavement Counselor who helps patients,
families and hospice staff understand the process of dying and
provides the support they need before and after death.
Volunteers who offer respite time for
caregivers, company for patients, assistance with household
chores and many other services. |
